It's definitely in Milan's Encyclopedia (sorry, I don't have that to hand at present). I also remember a brief but extremely useful article by then editor Colin Vaughan in an issue of "The Problemist" from the (mid?) 1970s that dealt with form, antiform, inverted form and inverted antiform. (Alas, that isn't to hand either, but it's probably available in PDF from the BCPS website. I'll investigate...) It might also be useful to consult a thread here on the MatPlus forum about Take&Make, where the question of its antiform arose...
